1.       Definition
a.       Tachypnea
Tachypnea  is the condition of rapid breathing. In adult humans at rest, any rate between 12-20 breaths per minute is normal and tachypnea is indicated by a ventilatory rate greater than 20 breaths per minute. Children have significantly higher resting ventilatory rates, which decline rapidly during the first three years of life and steadily until around 18 years.
b.      Bradhypnea
Bradypnea is an abnormally slow breathing rate. The rate at which bradypnea is diagnosed depends upon the age of the patient.
c.       Tachycardia = Too fast
Tachycardia is a  heart rate of moreyzthan 100 beats per minute (BPM) in adults. Tachycardia can be caused by various factors which often are benign. However, tachycardia can be dangerous depending on the speed and type of rhythm
d.      Bradycardia
Bradycardia is a condition of the resting heart rate of under 60 beats per minute, though it is seldom symptomatic until the rate drops below 50 beats/min.

3. The Conversation Between Nuse and Patient About Vital Sign
In Polyclinic
Nurse    : Good Morning Madam. Is there something that I can do for you?
Madam                : Morning. I want to get medical check up. Can you help me?
Nurse    : Before get medical check up, you must get register before, Madam.
Madam                : Ockay, thank you Nurse.
Nurse    : You’re welcome.
                The Patient get register. After have registered, Madam Padrista back and ask a question.
Madam                : Good morning Nurse. I have registered to get medical check up. What should I do right now?
Nurse    : Madam can wait on the chair till we call Madam’s name. and then, Madam can do the medical check up.
Madam                : Ockay Nurse, thank you.
Nurse    : Your welcome Madam.
                The time for Madam Padrista are comins. She is called by a Nurse.
Nurse    : Madam Padrista?
Madam                : Yes, me.
Nurse    : You can do the medical check up Madam. But, before it, Madam must checking Madam’s vital sign first.
Madam                : Ockay. Where the checking will do?
Nurse    : the checking will do in checking room, Madam. You can accompany me to the checking room. This way Madam.
Madam                : Ockay, thank you.
                Madam and Nurse go to the checking room and Madam si on the chair.
Nurse    : Ockay, Madam….?
Madam                : Padrista. My name is Padrista.
Nurse    : Good morning Madam. I’m Ns. Fanny. How are you this morning?
Madam                : Morning Ns. Fanny. I get a headache this morning ang want to get medical check up.
Nurse    : Ockay Madam, would you like to check your vital sign before you get your medical check up? It’s important to know about Madam’s condition, is it on normal number or not.
Madam                : Oh, sure. With pleasure.
Nurse    : Madam, which position do you feel comfort? Do you want to lie or sit down?
Madam                : I think I want to lie, because it’s more comfort than sit down.
Nurse    : Ockay Madam, before we check your vital sign, I’ll explain about procedures.  The first, I want to check your pulse. Is it regular or not and to make sure that it’s on normally number. The themperature will we check together with checking pulse. Is it at high number (hypertherm) or lower number (hypotherm). And the last, we will check your blood pressure. Is it at normally number or not. Is it clear for you, Madam? Are there any questions to ask? Do you agree with the procedure?
Madam                : Yeah, it’s clear. I agree with all procedures. Thank you, Nurse.
Nurse    : Now, we begin the procedure.
                The nurse begin the procedure and prepare all the kit.
Nurse    : Excuse me Madam, can I put the thermometer in your armpit and hold your wrist?
Madam                : Of course.
                After a while, the result can be read. And the nurse continue to count the respiration rate without known by the patient. After that, Nurse checks Madam Padrista’s blood pressure.
Nurse    : Ockay Madam, we continue to the last checking. Blood pressure. The manset will around your arm, exactly at biccep and trisep. Then, I’ll make a pressure by pump to the manset. After that, I’ll relax it slowly.
Madam: Ockay.
                The nurse pump the manset and listen to the first beat and the last beat as sistol and diastole. And relax it back slowly. And give the result from the checking.
Nurse    : Madam, the checking was finished.
Madam: How about the result, Nurse?
Nurse    : The result show that you are in a good condition. Your vital sign number was normal, with blood pressure 120/80 mmHg, and pulse was 82 bit per minute (82 bpm), then the themperaturre shows 36.8˚C. Now, Madam can continue the medical check up.
Madam                : Thank you Nurse          
Nurse    : You’re welcome. Nice to meet you. See you.
Madam                : Nice to meet you too, see you.
                Madam Padrista left the vital sign checking and continue to medical checking. The nurse back to her activities, to check another people too.
